Chrome users who have updated their browsers to version 142 (and reintroduced in version 143) may be asked to update their Local Network Access Permissions, and if you accept the default your access to Banner forms is disrupted.
To resolve this issue in Chrome, you can navigate to this url chrome://flags/#local-network-access-check and change the value from default to disabled.
You will need to re-launch the browser and your access will be resolved. Closing the tab is not sufficient to re-launch the application.
